How to Register for PhilHealth YAKAP: Step-by-Step Guide (2026)

To register for PhilHealth YAKAP, you follow four steps in order: (1) make sure you have an active PhilHealth membership, (2) select a YAKAP clinic through the eGov PH Super App, the PhilHealth Member Portal, or any PhilHealth office, (3) visit that clinic for your First Patient Encounter (FPE), and (4) sign the YAKAP Empanelment Slip (YES). Only after all four steps are you "empaneled" — officially enrolled — and able to use free consultations, laboratory tests, medicines, and cancer screenings.

That four-step path — Registration, Selection, First Patient Encounter, Empanelment — comes straight from PhilHealth Circular No. 2025-0017, the rulebook governing YAKAP. This guide walks through each step in plain language.

Who Can Register for YAKAP?

Every PhilHealth member and their qualified dependents can register. The circular defines a YAKAP beneficiary as any individual — member or qualified dependent — eligible to avail of PhilHealth benefits. The official YAKAP page lists 12 covered member categories, including:

Qualified dependents include your legitimate spouse (if not a member), unmarried and unemployed children below 21, children 21 or older with a disability that makes them totally dependent, foster children, and parents aged 60+ with income below the PhilHealth threshold — provided they are declared in your Member Data Record, per PhilHealth's qualified dependents page. One important rule: for minors, a parent or legal guardian must handle all YAKAP transactions — registration, clinic selection, and signing. Beneficiaries 18 and above transact on their own. Step 1: Make Sure Your PhilHealth Membership Is Active

Before anything else, you need a PhilHealth Identification Number (PIN). If you're not yet a member, PhilHealth's official registration steps say to get your PIN online through the Member Portal or at any Local Health Insurance Office (LHIO). If you already have a PIN, update your records so your address and dependents are current — your declared address matters for clinic selection (see the 30-km rule below).

Also worth knowing: clinics verify your identity through the PhilHealth Check Utility (PCU), an online tool that confirms your PhilSys registration and membership using your details and a facial-recognition "liveness check." If you're not registered in PhilSys (the national ID system), bring a valid government-issued ID so the clinic can validate you manually.

Step 2: Choose Your YAKAP Clinic

You can select your clinic through any of six official channels listed in Circular 2025-0017:

  1. The eGov PH Super App
  2. The PhilHealth Member Portal
  3. Any PhilHealth office (walk-in at an LHIO with a Primary Care Selection Form)
  4. The YAKAP clinic itself
  5. The PhilHealth Corporate Action Center (phone or email, with the form and a government ID)
  6. P-CARES staff stationed in hospitals

In the app or portal, you browse accredited clinics, pick one, and confirm. Two rules protect you here. First, per PhilHealth Advisory 2026-0023, your clinic must be a fixed, permanent, accredited facility within a 30-kilometer radius of your declared home or office address — you may choose one farther away if that's your clear, informed preference, but PhilHealth strongly discourages it, and mobile units or telemedicine-only providers cannot be your YAKAP clinic. Step 3: Complete Your First Patient Encounter (FPE)

Selecting a clinic online does not activate your benefits — the FPE does. The FPE is your first visit of the year, where the clinic takes or updates your basic health data to identify your health risks and build your profile. Circular 2025-0017 is explicit: the FPE is not a medical consultation. It's a health-risk assessment, and it can be conducted by doctors, nurses, midwives, barangay health workers, or other trained clinic personnel — not only physicians.

What happens on FPE day, based on PhilHealth Advisory 2025-0071:

  1. You undergo a PCU liveness check (a quick facial scan) to verify your identity
  2. Clinic staff record or update your basic health information
  3. You sign the YAKAP Empanelment Slip

Without an FPE, you cannot access any YAKAP benefits — and if you selected a clinic but never showed up for your FPE, you won't be retained by that clinic the following year and will have to select again.

Step 4: Sign the YES — You're Now Empaneled

Empanelment is officially defined as the formal care relationship between you and your chosen clinic after completing the FPE. It becomes official when you sign the YAKAP Empanelment Slip (YES), also called the Mutual Care Agreement (MCA) — required for all empanelments since January 1, 2026, and re-signed annually. As of PhilHealth Advisory 2026-0029, the YES is digital: after the clinic explains the agreement, you do a second PCU liveness check, which serves as your digital signature. You can request a printed copy.

Once empaneled, you belong to that clinic for at least a year. It becomes your first point of contact, care coordinator, and provider of complete primary care services. Want to switch later?
