Yes, you can transfer to a different YAKAP clinic — but the timing depends on whether you are already empaneled. If you selected a clinic but have not yet completed your First Patient Encounter (FPE), you can transfer anytime by visiting the nearest PhilHealth Local Health Insurance Office (LHIO), effective immediately. If you are already empaneled, you can only transfer during the fourth quarter of the year (October to December), and the transfer takes effect in January of the following year.
These rules come from PhilHealth Circular No. 2025-0017, the governing circular for YAKAP selection and empanelment. Let's walk through what they mean for you in plain language.
First, What Does "Empaneled" Mean?
Empanelment is the formal care relationship between you and your chosen YAKAP clinic, and it only begins after you complete your First Patient Encounter. Under PhilHealth's rules, availing the primary care benefit follows four steps in order:
- Registration — you register or update your PhilHealth records and get your PhilHealth Identification Number (PIN).
- Selection — you choose your YAKAP clinic.
- First Patient Encounter (FPE) — the clinic takes or updates your basic health data to identify health risks. Note that the FPE is officially not a medical consultation — it's a health-profiling visit.
- Empanelment — after the FPE, you sign the YAKAP Empanelment Slip (YES), also called the Mutual Care Agreement (MCA), and you are formally empaneled.
Once empaneled, you are considered attached to that clinic for at least a year unless you transfer. Your clinic becomes your first point of contact, your care navigator, and your provider of complete primary care services. This is why PhilHealth doesn't allow empaneled members to hop between clinics mid-year — empanelment is designed for continuity of care, not a one-time health encounter.

When Are Transfers Allowed?
There are two transfer tracks, and one set of exceptions.
Track 1: You selected a clinic but haven't done your FPE yet
You can transfer anytime during the year. Visit the nearest LHIO (any PhilHealth office), request the transfer, and it takes effect immediately after processing. This is the flexible window — if you registered with a clinic and changed your mind before your first visit, switching is quick and painless.
Track 2: You are already empaneled (FPE done, YES signed)
You can only transfer during the fourth quarter of the year (October–December), and your transfer takes effect in January of the following year. Until then, your current clinic remains your primary care home, and you keep availing your benefits there.
The exceptions: mid-year transfers for empaneled members
Per PhilHealth Circular No. 2025-0017, an empaneled beneficiary may transfer before Q4 only under three circumstances:
- The clinic closes (facility closure).
- The clinic's PhilHealth accreditation is revoked, suspended, or withdrawn.
- The Department of Health issues a cease-and-desist order against the clinic.
In these cases, you are not stuck — PhilHealth lets you move to a new accredited clinic so your care isn't interrupted.
How to Process the Transfer
Today, the primary way to transfer is to visit the nearest LHIO. Bring a valid government-issued ID; if you're transacting for a minor dependent, a parent or legal guardian must do it on the child's behalf (beneficiaries 18 and above transact independently).
PhilHealth has also committed to adding transfer functions to all its clinic-selection platforms — the same channels you used to pick your clinic in the first place:
- The eGov PH Super App
- The PhilHealth Member Portal
- Any PhilHealth office (LHIO) — currently the main transfer channel
- The YAKAP clinic itself
- The PhilHealth Corporate Action Center (phone or email, with a Primary Care Selection Form and government ID)
- P-CARES staff stationed in hospitals
Once those platforms are enhanced, online self-service will become the primary method for clinic transfers. Until PhilHealth announces that rollout, plan on an LHIO visit.
A few practical rules to remember when choosing your new clinic:
- Pick from the official list of accredited YAKAP clinics, which PhilHealth updates periodically.
- Your clinic should be within a 30-kilometer radius of your declared home or office address. Choosing farther is allowed if it's your clear, informed preference — but PhilHealth strongly discourages it.
- Your new clinic cannot reject you unless it has reached its declared beneficiary limit.
- Only fixed, permanent, accredited clinics count — mobile health units and telemedicine-only providers cannot be the basis of valid empanelment.
What Happens to Your Records and Benefits?
After transferring, you restart the empanelment process at your new clinic: a PhilHealth Check Utility (PCU) liveness check, a new FPE, and a new YES/MCA. Since Advisory No. 2026-0029, the digital YES via the clinic's HCI Portal is the primary and required platform for empanelment — your second PCU liveness check serves as your digital signature, and you can request a printed copy.
Key effects to expect:
- Benefits continue, but at the new clinic. Your PhilHealth membership and YAKAP entitlements don't reset — you simply avail them through your new primary care home once empaneled there.
- An empaneled transfer takes effect in January. If you file in Q4, keep using your current clinic for consultations, labs, and prescriptions until the new year.
- Your health data follows through the FPE. The new clinic takes and updates your basic health profile during your FPE, rebuilding your record in its EMR system.
- Annual renewal applies either way. Even if you don't transfer, you'll sign a new YES/MCA annually to signify continued empanelment. Members with a completed FPE are automatically retained by their clinic for the following year unless they transferred in Q4; members who selected a clinic but never completed the FPE are not retained and must re-select.
That last point matters: skipping your FPE means losing your clinic slot at year-end — and without an FPE you can't access any YAKAP benefits at all, as PhilHealth's member advisory on the YES makes clear.
