What Does 'Empanelment' Mean in PhilHealth YAKAP?

Empanelment means you are officially "adopted" as a patient by the YAKAP clinic you chose — a formal care relationship between you and that clinic, established after you complete your First Patient Encounter (FPE). Once you are empaneled, that clinic becomes your health home for at least a year, and your free YAKAP benefits — checkups, laboratory tests, medicines, and cancer screenings — become available to you.

Empanelment, Defined Simply

Empanelment is PhilHealth's official term for the moment a clinic formally becomes *your* clinic. PhilHealth Circular No. 2025-0017 defines it as "the formal care relationship established between beneficiaries and their chosen Primary Care Clinics after completion of First Patient Encounter (FPE)." It creates mutual accountability: you commit to that clinic as your first stop for care, and the clinic commits to look after you.

Think of it like enrolling in a school. Picking a school is not the same as being enrolled — you still need to show up, submit your records, and sign the enrollment form. YAKAP works the same way, in four steps set by the circular:

Registration — you have a PhilHealth Identification Number (PIN) and updated records.
Selection — you choose your YAKAP clinic (via the eGov PH app, Member Portal, any PhilHealth office, or the clinic itself).
First Patient Encounter (FPE) — the clinic takes or updates your basic health information.
Empanelment — you sign the agreement, and the care relationship becomes official.

You can only be validly empaneled to a fixed, permanent, accredited YAKAP clinic — not a mobile unit or a telemedicine-only provider — and the clinic should normally be within 30 kilometers of your home or workplace.

What Is the YAKAP Empanelment Slip (YES)?

The YAKAP Empanelment Slip, or YES, is the document you sign to make your empanelment official. It is the same thing as the Mutual Care Agreement (MCA) — two names for one document, per PhilHealth Advisory No. 2025-0071. The clinic gives it to you after your FPE, and signing it confirms your active care relationship.

Key facts about the YES:

It's required. Mutual Care Agreements became mandatory for empanelment starting January 1, 2026, under PhilHealth Circular No. 2025-0017.
It renews yearly. You'll be asked to sign a new one each year to signify continued empanelment.
It's a two-way promise. The clinic commits to provide your covered services for the calendar year; you commit to it as your primary care home.
It's now digital. Since PhilHealth Advisory No. 2026-0029, the digital YES via PhilHealth's HCI Portal is the primary and required platform for empanelment. Instead of a pen, you do a second PCU liveness check — a quick facial-recognition verification — which serves as your digital signature. You can ask for a printed copy if you want one.
For minors, a parent signs. All official YAKAP transactions for beneficiaries below 18 — registration, clinic selection, and signing the agreement — are done by a parent or legal guardian.

Why the FPE Completes Your Empanelment

The FPE is the visit that turns your clinic selection into a real care relationship — without it, empanelment simply never happens, and you cannot access any YAKAP benefits. Here's the surprising part: the FPE is not a medical consultation. Circular 2025-0017 says so explicitly. It is your first patient contact for the year, where the clinic takes or updates your basic health data to identify your health risks — think of it as the clinic getting to know you before it starts caring for you.

The usual flow on FPE day, based on PhilHealth's member advisory:

You arrive at your chosen clinic (schedule ahead if you can).
Staff run a PCU liveness check to verify your identity.
Clinic personnel complete your FPE — height, weight, history, risk screening.
The YES terms are explained to you, and you sign (digitally, via a second liveness check).
You are now officially empaneled.

Why does PhilHealth insist on this step? Two practical reasons:

Your benefits depend on it. Free consultations, the 13 laboratory tests, GAMOT medicines worth up to P20,000 per year, and cancer screenings all flow through your empaneled clinic. Retention depends on it. If you selected a clinic but never completed the FPE, PhilHealth will not retain you at that clinic the following year — you'd have to select all over again. Empaneled members, by contrast, are automatically retained unless they transfer.

How Long Empanelment Lasts — and Can You Change Clinics?

You stay empaneled to your clinic for at least a year, and your clinic cannot drop you. Circular 2025-0017 bars clinics from rejecting beneficiaries who selected them, unless the clinic has reached its declared patient limit. Your clinic serves as your first contact, navigator, and coordinator — and other PhilHealth-accredited facilities are required to refer you back to it after treating you.

Switching is possible, with timing rules:

Before your FPE — transfer anytime at the nearest PhilHealth Local Health Insurance Office (LHIO), effective immediately.
After empanelment — transfers are allowed only in the fourth quarter of the year, taking effect the following January. Exceptions apply if your clinic closes, loses accreditation, or receives a DOH cease-and-desist order.
