The YAKAP Empanelment Slip (YES): When You Need It and How It Works

The YAKAP Empanelment Slip (YES) is the signed document that certifies a beneficiary has completed the First Patient Encounter (FPE) and is formally empaneled — assigned as a patient — to your Primary Care (PC) Clinic for the coverage year. It is issued right after the FPE, and it must be signed by both the beneficiary (or their parent/guardian, if a minor) and an authorized representative of the clinic.

What the YES Certifies

The YES certifies that a specific beneficiary underwent an FPE at your clinic and has entered into a Mutual Care Agreement (MCA) with that clinic for primary care coverage. PhilHealth Circular No. 2025-0017 frames this as a mutual acknowledgment: after the FPE, the beneficiary and the PC Clinic both confirm through the Mutual Care Agreement that they acknowledge mutual responsibilities under the primary care benefit, and PC Clinics must submit signed MCAs as a requirement for first-tranche payments. In practice, the YES *is* this MCA artifact for empanelment purposes — PhilHealth Advisory 2025-0072 refers to it directly as the "MCA/YES."

Once signed, the beneficiary is empaneled to your clinic for at least a year. Empaneled beneficiaries generally cannot transfer to another YAKAP Clinic mid-year — transfers are restricted to the fourth quarter (effective the following January), except in cases of facility closure, loss of accreditation, or a DOH cease-and-desist order.

When Is the YES Issued

The YES is issued immediately after the FPE — never before it, and never as a substitute for it. The FPE itself is explicitly not a medical consultation; PhilHealth Circular No. 2025-0017 defines it as "the initial episode of patient contact for the year whereby a PC Clinic takes and/or updates the basic health data of an eligible beneficiary to identify their health risk," and states plainly that "the FPE is not a medical consultation."

The full availment sequence, per the same circular, runs in four steps:

Registration — the beneficiary registers to the KPP (a YAKAP Clinic package).
Selection — the beneficiary selects their PC Clinic.
First Patient Encounter — health data is taken or updated.
Empanelment — the YES/MCA is signed, formally attaching the beneficiary to that clinic.

Registration doesn't need to happen on a separate visit — it can be done the same day as the FPE, and an FPE performed before formal registration remains valid as long as the beneficiary registers to the KPP within the applicable year. A signed MCA/YES is required annually, not just once. Rolled-over members from the prior year must sign a new MCA/YES again in the current year — the empanelment doesn't carry over automatically.

Who Signs the YES

Two parties sign the YES: the beneficiary (or their authorized signatory) and the PC Clinic's authorized representative. For beneficiaries who are minors, PhilHealth Circular No. 2025-0017 is explicit that all official transactions, including execution of the MCA, must be undertaken by a parent or legal guardian — beneficiaries 18 and above sign for themselves.

On the clinic side, whoever conducts or oversees the FPE — commonly the physician, a nurse, or another authorized staff member acting on the clinic's behalf — countersigns to confirm the encounter took place and the terms of the mutual care arrangement are accepted.

Why the YES Matters for SAP1 Payment

Without a signed YES, PhilHealth can delay or withhold your capitation payment. Effective January 1, 2026, every SAP1 submission must have an attached PCU Transaction Number and MCA — PhilHealth Advisory 2025-0072 warns that claims missing either "will likely encounter delays in processing or non-payment." For FPE records specifically, the requirement is even more granular: PhilHealth Circular No. 2025-0017 states that "PhilHealth shall require the PCU reference number, verification result, and date/time stamp, and signed MCA for FPE records" — meaning the YES doesn't stand alone, it's paired with a PCU liveness-check log for every FPE you bill.

Rolled-Over Members: The June 30, 2026 Deadline

Clinics carrying beneficiaries forward from CY2025 have a hard compliance deadline: MCA and PCU Liveness Check compliance must be completed on or before June 30, 2026. Beyond that date, per PhilHealth Advisory 2025-0072, clinics must return SAP1 payments for every rolled-over beneficiary that lacks a signed MCA, subject to PhilHealth Payment Recovery.
